Please fill out the appropriate repair form below. If you are unable to, please notify the front desk that you were unable to fill out this form and they will fill this out for you:

Elementary

For our Online Learning and Elementary Students: Please return the broken Chromebook to the front office of the building you are attending. The front office will then give the student a loaner Chromebook if one is available. 

For our In-Person Jr/Sr High Students: Please return the broken Chromebook to the Jr/Sr library. The student will receive a loaner Chromebook if one is available.

Once the repair is completed, you will be notified to return the loaner Chromebook if applicable and where to collect the repaired Chromebook.
