2024-2025 Free Athletic Physicals
Where: Mount Pleasant Area Jr/Sr High School Field House
When: Friday June 7, 2024 10am-2pm
Grade 7: 10:00am - 10:40am
Grade 8: 10:40 am - 11:20am
Grade 9: 11:20 am - 12:00 pm
Grade 10: 12:00pm - 12:40pm
Grade 11:12:40 pm - 1:20pm
Grade 12: 1:20 pm - 2:00 pm
How: Student Athletes sign up on the Google Form. Link will be shared on the athletic website and sent to the Student Athletes school email address.Please sign up by the last day of school. Siblings in different grades may go during the same time.
Arrive during your grade’s time with pages 1-5, 7,10-12 of PIAA CIPPE filled out and signed by both Parents/Guardian and Athlete.
Physical Copies will be in the Jr/Sr High Office
Physical copies can also be printed off from the Athletic website.
Athletes will not be seen unless the designated sections of PIAA CIPPE packet is filled out.
Athletes who have CIPPE completely filled out will be seen before someone who arrives with it not filled out.
Come with the packet filled out.
Concussion Baseline Testing:
Athletes going into 7th, 9th, and 11th or new to a sport /Mount Pleasant will need an updated concussion baseline.
The last question on the google form has a box for anyone going into 7th, 9th or 11th grade or new to sports to put an email address in for a link to be sent home for the baseline.
